Hose and coupling



  • Hoses and coupling are mainly used in Oil & Gas plant for: 
Utility stations: supply water, plant air, steam and N2
– Hydraulic oil: torquing operation etc.
– Fuel and Chemical transfer


  • There is a risk of exposure
 

Some HAZARDS related to hoses & coupling involve:
  1. Wrong hoses / coupling used
  2. Substandard fittings & hoses
  3. Incompatibility
  4. Temporary arrangements
  5. Misuse
  6. Lack of inspection & maintenance
  7. Failure to renew in time (old materials)
  8. Housekeeping and improper storage
 

Example of incidents involving hoses & coupling:


 

The root cause for these incidents: FAILURE to follow recommendations.
INCIDENTS related to hose and coupling failure. 
A pressurized air hose disconnected from torque machine coupling during operation. 
No injury reported.

Preventative measures and safe practices:
  1.         Inspection of hoses and associated couplings prior starting.
  2.         Use of standard connections and hoses that met requirement.
  3.         Always use anti-whip slings for all pressurized hose connections.
  4.         Removed all defective hoses & couplings immediately and replace by proper one.
  5.         Use of jubilee clamps is PROHIBITED
